Official Petition Form

Below you will find a link to the official petition form. This form can be used for signature gathering. Please send completed forms to the address below. If you download this form to print and collect signatures with, please keep the following in mind:

  • The form must be printed double sided. This is a legal requirement.
  • The form must be printed exactly at this size. It is a requirement to keep the one-inch margin at the top of the first page. Adobe Acrobat reader sometimes shrinks the printout when printing. Please set page scaling to None. Click here for an example.
  • Please use a laser printer or photocopier. Forms printed on inkjet printers will smear if they get wet, and that may result in all the signatures being invalidated.
  • When mailing us the forms, remember that a standard first class stamp is enough for mailing five sheets of paper in a standard business envelope, if nothing else is added.
  • Our signature gathering deadline is September 27, although we would really like to have signatures come in as people collect them, because we need to know how far we are from reaching our goal.
  • If you download a petition, please send us an email so we can better track our progress and send you important reminders as the deadline approaches.
